Ferritin Test: The Iron-Store Report Your Normal CBC Is Hiding

Haemoglobin normal, yet hair fall, tiredness and weakness continue? The missing number is often ferritin — your body's iron store. Here is what the ferritin test shows that a CBC cannot.

"My CBC is normal, doctor, then why am I so tired?" We hear some version of this regularly, most often from women — hair falling in bunches, energy gone by afternoon, yet every report stamped normal. Very often, the one test nobody ordered holds the answer: ferritin.

What is ferritin, in simple words?

Think of iron in your body like stock in a godown. Haemoglobin is the cash moving in the market today; ferritin is the stock lying in storage. Ferritin is a protein that stores iron, and its level in blood shows how full or empty your iron godown is. The godown empties long before the daily cash runs out — meaning ferritin falls first, months before haemoglobin drops and anaemia appears on a CBC.

Signs your iron store may be empty

Constant tiredness that sleep doesn't fix, unusual hair fall, pale skin, brittle nails, restless legs at night, getting breathless on stairs you once climbed easily. In women, heavy periods are the commonest drain; in others, it can be poor diet, acidity medicines taken for years, or slow blood loss the doctor will want to investigate. Low ferritin is a finding, not the full diagnosis — the reason behind it matters.

One trap to know: fever and infection

Ferritin also rises when the body is fighting an infection or inflammation — it can look healthy or even high in a report done during a fever, while the stores are actually low. This is why doctors often read ferritin together with your symptoms, CBC and sometimes a CRP report, not alone. If your ferritin was checked during an illness, tell your doctor; the timing changes the meaning.

Who should get it tested?

Anyone with unexplained fatigue or hair fall and a "normal" CBC, women with heavy periods, pregnant women, vegetarians with weakness, and patients whose doctor is working up anaemia. No fasting is usually needed, and the report at our centre usually comes the same day. If your last report said "all normal" but you don't feel normal, bring it to us — or WhatsApp it to +91 9451155402 — and we will tell you plainly whether ferritin is the missing line in it.
