IGRA Blood Test for TB: What the QuantiFERON Test Can and Cannot Tell You

A household member has TB, or a cough won't settle — and the doctor writes IGRA, the QuantiFERON-TB Gold blood test. How it differs from the old skin test, and what a positive result really means.
When TB enters a household, the questions start immediately: should the rest of the family be tested? The older answer was the Mantoux skin test — an injection on the forearm and a return visit two days later. The newer answer is a blood test: IGRA, most commonly the QuantiFERON-TB Gold. Here is what it does well, and the one thing it cannot do.
How a blood test detects TB
IGRA checks whether your immune system has met the TB bacteria before. A blood sample is exposed to TB-specific proteins in the lab; if your immune cells recognise them — because they have fought this bacteria earlier — they release a measurable signal (interferon-gamma, which is where the test gets its name). No recognition, no signal: negative. Strong recognition: positive — your body has encountered TB at some point.
Why doctors prefer it over the skin test in India
Almost all of us received the BCG vaccine in childhood — and BCG can make the Mantoux skin test show positive even without TB. IGRA is not fooled by BCG, which makes it far more specific for India. It also needs a single visit — no returning after 48–72 hours to have a skin bump measured, and no reader-to-reader variation in judging that bump.
The one thing it cannot tell you
A positive IGRA means TB infection — it does NOT say whether the bacteria are asleep (latent infection, no illness, not contagious) or awake and causing disease (active TB). That difference is decided by symptoms, examination and a chest X-ray, not by this number. A negative result is reassuring, though in very early infection or very weak immunity the doctor may still keep watch. So treat the report as a strong clue placed in your doctor's hands — never as a final verdict by itself.
Who is usually advised this test
Family members and close contacts of a TB patient, people starting medicines that suppress immunity, healthcare workers, and patients whose long-standing cough or fever needs a careful workup. At our centre the QuantiFERON-TB Gold sample is taken through the day and the report usually comes the next day.
