LFT Enzymes Explained: SGOT, SGPT, GGTP and What They Mean

Fatty liver on the ultrasound, SGOT slightly up, GGTP higher still — the liver report raises more questions than it answers. Here is what SGOT, SGPT and GGTP actually measure, in plain words.
Few reports create as much quiet worry as an LFT. The ultrasound says "fatty liver", the blood report shows SGOT and SGPT mildly up, GGTP higher — and by evening the family has stopped using oil at home. Before the kitchen gets punished, it helps to know what these three lines actually are.
SGOT and SGPT: the leak from liver cells
SGOT (also called AST) and SGPT (also called ALT) are enzymes that normally live inside liver cells, doing their work quietly. When liver cells are irritated or damaged, the enzymes leak into the blood and the report rises. The commonest cause today is not alcohol or a virus — it is fatty liver from weight, sugar and a desk-bound life. Medicines, viral hepatitis and alcohol are the other usual suspects. SGPT is the more liver-specific of the two; SGOT also comes from muscle and heart, which is why doctors read the pair together, not alone.
GGTP: the line that points at alcohol and the bile ducts
GGTP (GGT) sits in the liver and bile passages, and it is famously sensitive to alcohol — in many reports it is the first line to rise in regular drinkers, sometimes before SGOT and SGPT move. It also rises when bile flow is disturbed. Patients sometimes ask us directly whether the doctor will "know" about drinking from the report. Honestly — a raised GGTP often starts that conversation, and it is better had openly, because the same number is also exactly how improvement shows: GGTP falls within weeks when the cause is removed.
How doctors read the pattern, not the panic
A mild rise — up to about twice the range — with fatty liver on ultrasound is extremely common and is usually a lifestyle correction story, tracked with a repeat LFT after some weeks. Values many times the range, rising bilirubin (the jaundice line), or symptoms like yellow eyes, dark urine and swelling send the doctor deeper, faster. What does not help: self-starting "liver tonics" from the chemist. They cloud the next report and treat nothing — the treatment for a raised enzyme is finding and removing its cause.
An LFT at our centre is reported the same day, and home collection is free across Lucknow. If your liver lines have come up abnormal, WhatsApp the report to +91 9451155402 — we will explain, line by line, which numbers look like lifestyle, which need a doctor soon, and when a simple repeat test is all that is required.
