Understanding LEVETIRACETAM, SERUM

This test measures the level of levetiracetam in your blood (serum). Levetiracetam — widely known by the brand name Keppra — is an anti-seizure medicine prescribed for epilepsy. The test needs a small blood sample (2 mL serum at this centre) and is analysed by LC-MS/MS, a highly specific laboratory method.

Levetiracetam behaves predictably in most people and has few interactions with other medicines, so unlike some older epilepsy drugs it does not need routine level checks for every patient. Testing is used in specific situations: seizures not controlled, doubt about missed doses, suspected side effects, pregnancy, and — importantly — kidney problems. Levetiracetam leaves the body mainly through the kidneys, largely unchanged; it is not significantly broken down by the liver. So when kidney function falls, the drug can build up, and doses are adjusted to kidney function.

The commonly used reference range for a trough levetiracetam level is about 12–46 µg/mL, but it is a guide, not a verdict: seizures can occur inside it and good control can exist outside it. Your treating doctor reads the level together with your seizure control, side effects, dose, kidney function and the exact timing of the sample. This test by itself cannot diagnose epilepsy or decide your dose — and you should never stop, skip or change levetiracetam because of a report.

When do doctors advise this test?

  • Seizures are not controlled on the current dose — the level helps show whether too little drug is reaching the blood, or doses are being missed
  • Doubt about adherence — a very low or undetectable level is important information for the treating doctor
  • Suspected effects of too much medicine — unusual sleepiness, dizziness, unsteadiness or behaviour change
  • Kidney function is impaired or changing — the drug is cleared by the kidneys, so levels can rise as kidney function falls
  • Pregnancy or pregnancy planning — levetiracetam levels fall as pregnancy advances, which can affect seizure control
  • Older patients and children, whose bodies clear the drug differently from typical adults
  • When the clinician wants a documented personal baseline level while you are well controlled, for comparison later

Preparation & the sample

No fasting is required for this test. Timing is what matters: the performing laboratory recommends the sample be taken just before your next dose is due, once you have been on a steady dose for a few days (steady state) — this lowest point is called the trough level. Note the exact time of your last dose and tell the collection team, and carry your prescription or medicine list. Do not skip or shift a dose on your own for the test — in most cases the sample is drawn just before the dose and the dose is taken right after collection; confirm the plan with your doctor or with us when booking. Call +91 9451155402 and we will help you schedule the collection at the right time.

This information is educational and general. Your doctor's advice for your specific situation always comes first.
