Understanding VALPROIC ACID; VALPROATE
This test measures how much valproic acid (valproate) is present in your blood. Valproate is a long-term prescription medicine used mainly to control epilepsy (seizures), and also for bipolar disorder and, in some patients, migraine prevention. The test needs a small blood sample (2 mL serum at this centre), is analysed by chemiluminescent immunoassay, and reports are issued daily.
Valproate is a medicine where the blood level genuinely matters — this is called therapeutic drug monitoring. Too little drug in the blood may fail to control seizures; too much raises the risk of side effects. The same dose can produce very different levels in different people, because age, liver function, pregnancy and other medicines all change how the body handles it. The commonly used therapeutic range for seizure control is about 50–100 µg/mL, though the reference range printed on your report and your doctor's judgement govern your case.
A single result never decides treatment on its own. Your treating doctor reads the level together with your seizure control or symptoms, any side effects, whether doses were missed, and the other medicines you take. Never stop, skip or change your valproate dose because of a report — dose changes are made by the treating doctor, usually gradually.
When do doctors advise this test?
- A few days after starting valproate, or after any dose change, to check the level has settled where the doctor wants it
- Seizures are returning, or control seems poorer than before
- Possible effects of too much medicine — unusual sleepiness, confusion, vomiting, marked tremor or unsteadiness
- Doubt about missed doses — the level helps the doctor judge adherence
- A new medicine has been started that can change valproate levels — certain antibiotics (such as carbapenems) and other epilepsy medicines are known examples
- Pregnancy or pregnancy planning, when valproate levels can shift significantly
- As a routine periodic check during long-term valproate treatment
Preparation & the sample
No fasting is needed for this test — but timing matters more than food. The sample is normally taken at the 'trough', just before your next scheduled dose, when the blood level is at its lowest and most comparable. Book your collection accordingly, and note the exact time of your last dose to tell the collection team. Do not skip or delay your dose on your own for the test — in most cases the sample is drawn just before the dose is due and the dose is taken right after collection; confirm the plan with your doctor or with us when booking. Carry a list of all medicines you take, since several can change valproate levels. Call +91 9451155402 and we will help you choose the right collection time.
This information is educational and general. Your doctor's advice for your specific situation always comes first.
